(Available on our website www.chts222.com.au) I agree to pay all fees and charges relating to my enrolment, unless payment is to be made by my employer or other contracted third party. I agree to maintain a minimum of 80% attendance. Learners who fall below this attendance rate will have to show why they should not be withdrawn.

I have been provided with information relating to this enrolment with Licensed Club Industry Training Foundation SA Inc t/a Complete Hospitality Training Skills, including the title and code of the Unit of Competency / Qualification, start and end dates, delivery mode/s, scheduled hours and assessment details. I acknowledge that Complete Hospitality Training Skills is required to retain my training records for a period of 30 years. I acknowledge and agree that in the event that CHTS ceases to operate or is under new ownership that those records will be transferred to either DSD or its quivalent at the time or to the new owners who will maintain the same or higher levels of security and privacy in regards to those records.

I give permission for Complete Hospitality Training Skills to post my Statement of Attainment / Certificate of Attendance to my employer if necessary.

Under the Data Provision Requirements 2012, Complete Hospitality Training Skills is required to collect personal information about you and to disclose that personal information to the National Centre for Vocational Education Research Ltd (NCVER). Your personal information (including the personal information contained on this enrolment form) may be used or disclosed by Complete Hospitality Training Skills for statistical, regulatory and research purposes. Complete Hospitality Training Skills may disclose your personal information for these purposes to:
- Commonwealth and State or Territory government departments and authorised agencies; and
- NCVER;
Personal information that has been disclosed to NCVER may be used or disclosed by NCVER for the following purposes:
- Populating authenticated VET transcripts;
- Facilitating statistics and research relating to education, including surveys and data linkage;
- Pre-populating RTO student enrolment forms;
- Understanding how the VET market operates, for policy, workforce planning and consumer information; and
- Administering VET, including program administration, regulation, monitoring and evaluation
You may receive a student survey, which may be administered by a government department or NCVER employee, agent or third party contractor or other authorised agencies. Please note you may opt out of the survey at the time of being contacted.
